Application submission dates for practitioner certification are accepted four times each year and should arrive in the HTI office during the 30 days proceeding the submission deadline:
      2010: January 15, April 15, July 15, and October 15
      2011: January 15, April 15, July 15, and October 15
      2012: January 15, April 15, July 15, and October 15
